Adult Community Health Services
Market Engagement December 2025
NHS Surrey Heartlands ICB is starting to consider the redesign of out-of-hospital care for adults in Surrey. This is a big opportunity to make services simpler, more joined-up, and focused on prevention.
Our thinking is at an early stage. Nothing is decided and we would like your ideas to help shape the future of local out-of-hospital services.
The redesign will align with:
- The NHS Long Term Plan and Surrey Heartlands Clinical Strategy
- National policy shifts toward neighbourhood-based care and population health management.
We are starting with a blank canvas. As part of our plan of engagement we will be reaching out widely to all stakeholders and the wider public over the coming months.
On 5th December 2025, we held an initial event with providers, hosted by Jack Wagstaff, Executive Director of Strategic Commissioning. This was the start of a collaborative approach with the market to service design and is the first in a series of anticipated virtual and face to face sessions with a range of stakeholders, details of which will be available on this page and promoted to the market on Find a Tender. Please watch this space for further information.
At our initial event, we asked service providers the following questions:
- What services in your view should be included in this model of care?
- How should the services be grouped or configured?
- What innovations can improve outcomes for people and service sustainability?
You can view the information provided at the event and key themes that arose from it here:
