NHS at Home

NHS at Home is an approach to providing better connected, more personalised care in people’s homes including care homes. It aims to ensure people have faster access to more appropriate and targeted care, without necessarily having to attend emergency care or arrange GP appointments.

  1. Blood Pressure at Home monitoring service BP at Home is a service which enables patients with high blood pressure (hypertension) to manage their blood pressure more effectively from the comfort of their own home.
  2. Virtual wards Virtual wards allow patients to get the care they need at home safely and conveniently, rather than being in hospital. The NHS is increasingly introducing virtual wards to support people at the place they call home, including care homes.
