Surrey Care Record
When you visit a healthcare professional, it’s vital that they can see all of your health information. Once they know more about you, such as your allergies, medications, diagnoses and treatments, they can better provide the care you need. This is especially the case if there is an emergency.
Improving your care
The Surrey Care Record, developed by clinicians themselves, supports your care by putting all your health information in one safe and secure digital place. With comprehensive information about your health immediately to hand, clinicians can then use this information to provide the essential and sometimes life-saving care you need.
For example, if you are admitted to hospital in an emergency, the A&E team can see your past treatment, any allergies or medications, and you won’t have to retell your medical condition. Your GP can look at test results or future appointments from your local hospital, and you won’t have to keep re-telling your medical history.
Only staff with appropriate authorisation have access to your information. And by using secure electronic systems, your information is held and shared more safely than the traditional methods of letters, forms, faxes and phone calls.

Data Sharing and Privacy Notice
This data sharing is now enabled by the Thames Valley and Surrey (TVS) Care Record system – please see Thames Valley and Surrey Care Records for further information.
The Surrey Care Record was therefore decommissioned in April 2025. If you have any specific queries or concerns regarding access to your personal or health data via the decommissioned Surrey Care Record, you should continue to send these to the Surrey Heartlands Information Governance team.
Only authorised care professionals can access the parts of your care record that helps them to deliver your care. Only approved staff can log onto the Surrey Care Record and all access is logged.
All organisations involved with the Surrey Care Record are legally required to explain how, why and with whom they share your health and care information, through a privacy notice.

Right to object
You have the right to object to the sharing of your data via the Surrey Care Record. However, we strongly recommend the sharing of data as real world experience demonstrates that it leads to higher quality and safer care. An objection to sharing your information will mean that your data will not be shared for any kind of direct care, including extended hours GP access and emergencies. If you do wish to object, you should therefore contact your GP practice and the other organisations involved in your care and understand what it means for you.
Working with Thames Valley and Surrey
The Surrey Care Record is also contributing to the wider health and care strategy across the wider Thames Valley and Surrey region, which will eventually lead to improved cross-border sharing of information, joining up health and care services people receive outside of their local area.
