The All Age Continuing Care (AACC) Patient Engagement Forum aims to enhance the patient experience by connecting service users with the AACC team to share insights and co-design solutions.

Established in 2019, the forum encourages participation from a diverse group including patients, family representatives, and voluntary organisations to address challenges in the NHS Continuing Healthcare processes.

The forum provides a platform for patients and their representatives to engage with healthcare professionals, ensuring that their voices are heard and incorporated into service delivery improvements.

The forum attracts individuals interested in understanding NHS Continuing Healthcare and the processes involved, such as the initial Checklist and Decision Support Tool (DST) Assessment.

Participants share their experiences, which help the AACC team identify challenges, address the diverse needs throughout the care planning stages and co-design solutions to improve the care process.
