Implementing our plans for people living with frailty in Guildford and Waverley
Guildford and Waverley Health and Care Alliance is delighted to share the Guildford and Waverley Frailty Strategy with our stakeholders and local communities. The publication of the Strategy means we can now begin to implement our plans and share our ambitions with a wider audience on our collective commitment to provide a consistent and coordinated approach to cross-organisation, multi-disciplinary, integrated delivery of health and care for people living with frailty.
We used valuable contributions from a series of stakeholder workshops earlier this year to shape a vision that captures all aspects of our Ageing Well programme. It is essential to gain the voice of our patients, carers and staff, and your feedback has enabled us to fully understand our current provision, identify gaps, and shape plans.
We will implement our plans over the next 3-5 years and continue to use opportunities for patients and carers to raise questions about the strategy and how we will turn our ambitions into reality.
