The Surrey Downs Health & Care Partnership (SDHCP) is a body consisting of the NHS, local government, community health services, voluntary sector and other providers.
The benefits of partnership working
Historically, different organisations that provide care to people in their homes, in the community, in GP surgeries and in hospitals have been limited by boundary lines, resulting in them working in a disjointed manner.
We realise the importance of coming together to ensure that local people receive the care that they need in the right way, in the right environment, and by the right individuals.
Through local partnerships, we will improve health and care for patients and residents of Surrey Downs, enabling them to access support more easily and quicker than ever before.

Our integrated teams across the Partnership are fully committed to working together to adapt our ways of working and implement changes to our services which serve our communities more efficiently, more conveniently and within our local area.
We’re proud of our work to date. Here are just a few examples and case studies of transformation projects which demonstrate partnership working within Surrey Downs, and that have delivered better outcomes for our patients and communities.
Surrey Downs Integrated Frailty System
Our integrated frailty system provides holistic, individualised care to residents requiring support at a time when they need it most.
The service is made up of a number of teams and settings who work closely together so that we can identify an individual living with frailty as early as possible, and then implement a support plan tailored to that person, with on-going support as required.
We consider a person’s time the most important currency and exploring what matters most to an individual is at the forefront of our working ethos. We are here to support people when in need and to promote independence.

Urgent Community Response Service
The Urgent Community Response Service provides an urgent response in the community to Surrey Downs residents, 18 years of age and over, seven days a week from 8am to 8pm. It is based within three areas across Surrey Downs: Epsom, East Elmbridge and Dorking.
This service is delivered by a local, multidisciplinary team and aims to give patients the most appropriate support in their home, or usual place of residence, to prevent unnecessary hospital admission.
